September 2017 – August 2018 Weekly Contribution Options The contribution amount is determined by the level of coverage the employee has selected. Below is an outline of the weekly contributions for the medical, dental, and visions plans. Paid Vacation (also known as Paid Time Off or PTO) is accrued based on the amount of hours an employee works from their date of hire with a maximum amount of PTO that can be earned within the calendar year, defined as January 1 – December 31. PTO accrual rates and minimum amounts are based on employee years of service: Working on a Holiday If you are asked to work on a holiday, you will receive time and a half for the work hours performed that day, in addition to any holiday pay that you would qualify for as noted on the left (this applies to all employees, regardless of tenure). However, if you agree to work on a holiday, and then call off on the day of the holiday, you would invalidate all forms of holiday pay. noun. a fixed amount of money paid by an insured employee for “out of the norm” services before the insurance plan provides further coverage. Copays and prescriptions usually do not count toward the deductible requirement. High Deductible = Lower Weekly Cost Our in-network medical deductible option amounts of $1,500, $3,000, and $6,000 may seem like a hard pill to swallow for some, but the following explanation may help it go down a bit easier. Flagger Force has opted to provide employees with two new higher deductible, lower weekly contribution plans based on our past history of insurance usage by our employees. Most people only meet their deductible amount when they have significant medical expenses like broken bones, surgery, major disease, or hospitalization. On average, only 20% of Flagger Force employees use up their deductible in a given year, meaning for most of our employees it is more beneficial to have lower weekly contributions than a lower deductible amount. Employees now have the option to choose the deductible amount that is right for them and their families needs. Additionally, if you have opted for employee + spouse or any form of family coverage, each person’s out-of-pocket, before-deductible medical expenses can be combined to meet the deductible amount. Months – 3 Years of Service Maximum of 40 hours can be accrued in a calendar year at the rate of 1 hour per every 40 hours worked. 3 Years – 5 Years of Service Maximum of 80 hours can be accrued in a calendar year at the rate of 2 hour per every 40 hours worked. 5+ Years of Service Maximum of 96 hours can be accrued in a calendar year at the rate of 2.5 hour per every 40 hours worked. • You must have completed a minimum of 6 months of service before you are eligible to use accrued PTO. • You can see your current available PTO hour on your paystub after 6 months of service. • Requests for time off must be submitted in writing, see you branch for a form. • Employment must be active at the time PTO is paid. • PTO must be used in full-day (8-hour) increments, Monday – Friday, excluding holidays. • A maximum of 24 hours of PTO may rollover to the next calendar year. • Any remaining PTO balance is forfeited at end of employment. • See your employee handbook for further details. Employees are eligible for paid holidays after 90 days of employment if they are of active status and qualify as noted below: • 8 hours paid per company holiday if 440 hours were accrued in the 13 weeks preceding the holiday • 4 hours paid per company holiday if 220 work hours were accrued in the 13 weeks preceding the holiday • Must be available to work on the day before and the day after the holiday to earn holiday pay, unless prescheduled time off has been requested and approved for those days.
